Davis Dental makes every effort to provide you with the best possible care as well as the most convenient financial options. To do so, we collaborate with you to maximise your dental insurance reimbursement for covered procedures. Please contact our staff if you have any problems or questions. It is critical to understand that your dental insurance policy is a legal agreement between you, your employer, and the insurance company. That contract does not include our office. The services and allowable fees chosen are determined by the cost of the policy to your employer and the negotiated arrangements with the dental insurance company, rather than by your dental office. Your dental coverage is not determined by what you require or what your dentist recommends. It is determined by how much your employer contributes to the plan. Employers typically pay some, but not all, of their employees’ dental expenses. Inform your employer if you are dissatisfied with the coverage provided by your insurance.

We are a PPO and do not participate in any HMOs. If you are unsure about the type of insurance you have, we recommend that you contact your insurance company. We pay the difference between our fees and those set by the insurance company when a dental office is considered a participating provider or “in network.” If you encounter a “out of network” provider, you may be responsible for the cost difference.

Dentemax has fee schedules for over 150 insurance companies. We are considered “in-network” for all carriers using their specific fee schedule because we participate with Dentemax. Your insurance company or human resources department should be able to tell you if your policy adheres to the Dentemax fee schedule.

We make every effort to provide you with the most accurate estimate based on information obtained from your insurance company; however, these estimates are subject to final approval by your insurance company and may change. No matter what your insurance coverage is, you are responsible for our office’s fees. All estimated co-payments and any applicable deductibles are due at the time of service and will be collected at the start of the procedure.

Davis Dental Group will submit a claim to your primary insurance company for services rendered as a courtesy. You are responsible for any portion of your bill that is not covered by insurance. Please keep in mind that any claim that is not paid within 45 days becomes your responsibility. It is critical that you keep us updated on any changes to your insurance plan.
